If you are a current student at JDHS, use this form to request your transcript.  The transcript will be mailed to the person or college you have designated on the form.  Be sure to complete all required fields.  Have all the information available before beginning the form.  There is no charge for current students. Students may use this form up until August 31st of the year they graduate.

***Former Students***

The transcript will be mailed to the person or college you have designated on the form.  Be sure to complete all required fields.  Have all the information available before beginning the form.  Total cost for this transcript request is $5.55.  Debit or credit card accepted.

***Colleges or Verification Companies ONLY***

If you are a college or verification company, use this form to request transcripts and education verifications. The transcript or education verification will be mailed to the address designated on the form.  If you prefer that it be emailed, please include that in the special instructions.  Be sure to complete all required fields.  Have all the information available before beginning the form.  Total cost for this request is $19.07.  Debit or credit card accepted.
